#:1: Complete and SIGN the Georgia Sports Agreement Terms
and Conditions form (see links below).
#2: Complete and SIGN the Credit Card Authorization form
(you may also call in the information).
or
Complete and SIGN the Billing Authorization form
(if you will be paying by check).
#3: Make a READABLE copy of your credit card (both sides) and
your photo ID (if you can’t read it, neither can we).
#4: Fax all completed documents back to Georgia Sports at
770-518-1326.
Once all documents are received, we will email you the following items:
A: An invoice / confirmation of your order.
B: Billing terms and due dates
C: Package shipping instructions
D: Package details and other information needed for your trip
